Hilchos Geirushin 3:13

Let’s say that a man is known by two names and his wife is known by two names. In a get, the man should write the names that he and his wife use most often and by which they are best known. A get should indicate that [the man] “or whatever names he is known by” divorces [the woman] “or whatever names she is known by.” If a get calls the husband or wife by a diminutive version of their name, it is valid.

Hilchos Geirushin 3:14

If a get uses only the less common name of the husband or wife plus “or whatever names (s)he is known by,” the get is invalid. If one of their names or the name of their city is written incorrectly, the get is invalid even though the get says “or whatever name (s)he is known by.”
