412. Forgetting to Remove Tefillin
Tefillin, Mezuzah v’Sefer Torah 4:20
If a person accidentally entered the restroom wearing his tefillin, he should cover them with his hand until he finishes his initial act of expelling waste matter, then leave the restroom, remove the tefillin, return, and finish relieving himself. This is because if one were to stop abruptly while in the act of expelling his waste matter, he might injure himself.

If one accidentally had marital relations while wearing tefillin, he should not handle the straps or the boxes until he washes his hands. Once he has done so, he should remove the tefillin. This restriction is because hands are active and touch a lot of things.
