4,031. Blemishes of the Nose and Lips
Hilchos Bias HaMikdash 8:7
There are six blemishes of the nose: (1) a sunken bridge, even if it’s not so sunken that he can put ointment in both eyes at the same time. This is the charum of Leviticus 21:18; (2) if the middle of the nose sticks up; (3) if the tip of the nose points down; (4) if the tip of the nose is bent; (5) if the nose is excessively big; (6) if the nose is excessively small. This is using the pinky; a nose that’s bigger or smaller than one’s pinky is considered a blemish.

There are three blemishes of the lips: (1) an upper lip that extends beyond the lower lip; (2) a lower lip that extends beyond the upper lip; (3) if the mouth hangs open so that saliva drips out.
