In some places, the accepted practice is that if an adhesion goes from a lobe to the flesh and the bones of the ribs, and the adhesion connects to both of them, the animal is prohibited. The Rambam’s father was one of those who prohibited this, but the Rambam himself permitted it. In very few places, they permit the animal even when the adhesion only connects to the bone, but the Rambam prohibited it.

In some places, the lungs are always inflated in order to see if they have a hole. In most places, however, they’re not inflated because there’s no reason to suspect an issue. The Rambam say that they never inflated lungs in Spain and in the West unless there was a reason to be concerned.
